Feedback forms
Customer feedback form
Make it easy for people to tell you what could be better. Keep it short, keep it friendly, and ask for more only when there’s more to say. A handful of short questions, one gentle follow-up, and nothing that feels like homework.

Questions worth asking
How was your experience?
One easy question first gets people started, and tells you the mood at a glance.
Tell us a little more
Shown only when someone says it could be better, which is exactly when you want the detail.
How would you rate us overall?
A few stars are quick to give and easy to compare from one month to the next.
How likely are you to recommend us to a friend?
A scale from 0 to 10 shows how strongly people feel, not just whether they’re happy.
What’s one thing we could do better?
Asking for one thing feels lighter than asking for everything, and the answers are sharper for it.
Your email, if you’d like a reply
Some people want to hear back. Everyone else can skip it.
How the form reads
Picture someone who has just used your service. The first question takes a second, and it asks nothing but how it went. If the answer is Lovely or Pretty good, the rest is quick stars and a scale, and they’re done. If it could have been better, a box opens and they can say why.
That is the moment feedback is most useful, and it’s easy to lose people there if the form asks too much, so the question comes only when it’s wanted. Read the answers together in the responses table, and look for the same complaint appearing twice. Once is a mood. Twice is a pattern.
Try answering it yourself as an unhappy customer would. If you can say what went wrong in a few honest lines, so can they.
A follow-up that helps
Show “Tell us a little more” when “How was your experience?” is “Could be better”. Happy customers finish in a moment; the others get space to explain.
Open the form yourself before you share it. Pick Could be better and check the box appears, then pick Lovely and check it goes away. Once it’s live, the responses table shows every answer, and you can export them as a CSV.
Keep it kind
- Keep the email optional, so people can be honest without being named.
- Thank people for the hard answers too. Someone who says it could be better is giving you a chance to fix it.
